Case Manager

 Job Title: Case Manager 


La Posada Providencia is an emergency shelter for immigrants in crisis who are seeking legal refuge in the United States of America. Responsible for advocating for clients, assessing needs, evaluating options, creating plans, and coordinating services. 

Essential Functions: 

1. Coordinates and facilitates services through assessment, evaluation, planning, and implementation. 

2. Communicates client needs to several staff members and follows up accordingly. 

3. Manage discharge plans upon exiting program. 

4. Works collaboratively with clients, families, attorneys, paralegals, social workers, doctors, counselors, etcetera to ensure high-quality services. 

5. Acts as the client’s advocate as it relates to human rights, privacy, and social justice. 

6. Maintains the client’s comprehensive case management record through detailed documentation. 

7. Coordinates with third-party service providers (e.g. ProBAR, Catholic Charities, and Refugee Services of Texas) on a regular basis. 

8. Collaborates in the grant and program compliance process by providing information used to populate monthly reports. 

9. Helps with the Street Outreach program. 

10. Other functions as needed 

Education and Experience: 

  • Minimum Bachelor’s degree in Social Work or related field 
  • License Preferred 
  • Human Services Experience 
  • Management or Supervisory experience 
  • Experience with disabilities helpful 
  • Valid Driver’s license 
  • Bilingual 

Supervisory Responsibility: 

  • Supervises Case Management Aide 
  • Supervises residents 

Reports To: 

  • Executive Director
